For a simple and functional Thumb-Bob, use a 2 - 56 or 4 - 40 socket head screw.  A spacer may be used to give additional height and mechanical advantage.  A section of threaded pivot pin may be used:  1/8" pivot pin for 2 - 56 socket head,  3/16" pivot pin for 4 - 40 socket head.  You can use as is, or fill the socket with colored epoxy.  File work the top with a needle file or jewelers saw, or drill out the socket and set an inlay of pearl, jade, etc.  For setting 3mm inlays, pre-drill socket head with 3/32" then 7/64" finish with 1/8".
